Impact

Allegheny Prenatal to Three partners collectively refer families:

  • to the ELRC to assist with accessing and affording high-quality child care

  • to Early Intervention for evaluation, service coordination, and developmental monitoring

  • to home visiting programs and family centers

  • to Hello Baby

Because of integrated referrals and a “no wrong door” approach to resources, Allegheny Prenatal to Three is proud to have achieved the following impacts over our first five years:

  • All families (11,000/year) receive outreach from Hello Baby and information directing them to an online resource hub

  • 25% of births whose families have higher levels of need receive proactive outreach and offers to enroll in services

  • 44% of the top 25% higher risk families enroll in one or more services

  • 300% increase in new referrals from the Early Learning Resource Center to WIC, Supplement Nutrition Assistance Program (SNAP), Early Intervention, home visiting, housing assistance, CHIP and Medicaid

We collectively review service usage data on an annual basis and reflect on the system-wide needs and how we can continuously improve to ensure we are meeting the needs of all families within Allegheny County.

“Working with my PN3 cross-system partners has taught me that the most important aspect of our work is rooted in relationships with each other, between our organizations, and our shared vision for families from pregnancy through age three and beyond."

— Dannai Wilson, Deputy Director, Bureau of Community and Family Health, Allegheny County Health Department
